Whether you currently refer patients to University Hospitals Home Care Services (UHHCS), are a new doctor to the UH Health System, or want to utilize a local, high quality home care provider, working with UHHCS is simple.

UHHCS is the one of the largest agencies in Ohio serving over 15,000 patients annually. UHHCS is Medicare certified and JCAHO accredited. You can access service 24 hours a day, 7 days a week, year round.

Continuity of Care Program

The Continuity of Care Program of UHHCS provides high quality, supportive care services to patients and their families. As patients and families move through their disease experience, the program's liaison will be there to guide and assist them across care settings.

The liaison collaborates with providers at UH, UHHCS, and Hospice of the Western Reserve to ensure that the changing needs and goals of patients are being met.

Program Eligibility:
  • Patients for whom care needs are anticipated due to a potentially life-limiting diagnosis
  • Patients who reside in Northeast Ohio
  • Patients who agree to accept services from UHHCS if home health care needs are identified during the course of their treatment

Private Duty

The Private Duty division of University Hospitals Home Care provides non-skilled home care for adult and pediatric clients from 2 to 24 hours a day, up to 7 days a week. UHHCS Private Duty service offers an affordable solution to individuals and couples who prefer to stay at home and receive quality care with minimal disruption to their lifestyle. Services are provided for short and long term to help support the family caregiver who must return to work or school, or may be in need of respite care.

Rehabilitation

University Hospitals Home Care offers comprehensive rehabilitation services including Physical, Occupational, and Speech Therapy.  Our primary goal is to help patients of all ages achieve an optimal level of functional independence.  Our therapists work in collaboration with physicians and other members of the home care team to make this goal a reality.
