The home nurse is your resource for a safe and rapid recovery. He or she will visit you at home to teach you - and your family or other caregivers - how to best meet your daily medical care needs. The nurse will also discuss a variety of topics, including safe administration of medication, wound care, as well as provide education about diet and activity.
Following a hospital stay, professional medical care is vital to helping you recover and get back on your feet again, or help you learn to live with an illness or disability. The overriding goal is to help you regain your independence by becoming self-sufficient and maintaining your optimum health.

Requesting Home Health Services
Although anyone may request home health care, all skilled services must be ordered by a physician. Patients, physicians, or family members can request services by calling 1-800-552-8442, 24 hours a day, seven days a week. You do not have to be in the hospital to request home health care services. Patients at home or those being cared for in an outpatient clinic, emergency room, or nursing home may be appropriate candidates for home health care services.
Our nursing staff, in collaboration with your physician, case manager or facility discharge planner, will provide you with the appropriate information, plan, and tools needed to continue your medical care at home. A member of our home health team will contact you to introduce themselves and arrange your first visit.

How and When do I ask for University Hospitals Home Care?
If your doctor has determined you need additional care outside of the medical facility, he/she or the facility social worker will give you the opportunity to choose a home care agency. Its at that time you can ask specifically for University Hospitals Home Care.
The doctor will then contact us to discuss your needs and design your care plan. Once you are discharged from the hospital, we will contact you or your caregiver to arrange the time for our nurse to come to your home and help you begin your recovery.

Will Medicare Cover Home Health Services?
Medicare will usually cover the following services for homebound patients:
- Skilled nursing care such as surgical wound care, respiratory care, nutrition therapy, injections, and selected infusion therapy.
- Home health aide services including help with personal care such as bathing, using the bathroom, or getting dressed.
- Rehab services administered at the recommendation of your doctor.
- Medical social services to help you with social and/or emotional concerns following your illness or surgery.
- Approved medical supplies, such as wound dressings.
- Medical equipment. Medicare usually pays 80% of the approved amount for necessary medical equipment, such as a wheelchair or walker.

Who is eligible for Home Nursing Services?
There are three criteria to qualify for home health care:
- The person must need either intermittent or part-time:
- Skilled nursing care
- Physical Therapy
- Occupational Therapy
- or Speech Therapy
- The person must be home bound
- The plan of care must be ordered and reviewed by a doctor

What if I do not have Medicare Coverage?
